Minnesota DHS requires a completed Health Care Summary (form MS-2083) for every child enrolled at Zayn Academy. This form must be filled out and signed by your child's healthcare provider — it cannot be completed by parents or guardians.
Download the MN DHS Health Care Summary form (MS-2083), bring it to your child's doctor's appointment, and have them complete and sign it.
